Nashville, Tenn. — EvidenceCare marks a decade of building clinical decision support that hospitals and physicians actually use.
Ten years ago, clinical performance improvement wasn’t a category. Now it’s table stakes for hospitals trying to hold margin while payers get harder to work with. EvidenceCare has spent the last decade building the tools that make it possible, and its founders think the next ten years matter more than the first.
Founded in 2016 by Dr. Brian Fengler and Jim Jamieson, EvidenceCare started with one idea: put the right evidence in front of physicians inside their EHR workflow, and the financial upside for the hospital follows the better decision. A decade later, that idea has scaled into a platform generating more than $100 million a year in improved revenue capture and reduced cost of care for its clients, with integrations across Epic, Oracle Cerner, and Meditech, and deployments in dozens of health systems and hundreds of hospitals.
It started with a physician frustrated by a broken process
Dr. Fengler was practicing emergency medicine when he ran into the gap that became EvidenceCare’s reason for existing: clinicians needed evidence-based guidance and best-practice treatment recommendations inside their workflow, and nothing built for that moment existed.
That gap became the company’s first product. EvidenceCare built clinical pathways in partnership with medical societies across emergency medicine, urgent care, orthopedics, and gastroenterology, and worked with leading cancer centers on oncology content. When COVID hit, the company opened access to its guidance so clinicians everywhere could work from best practices that were changing week to week.
From clinical guidance to margin impact
COVID changed what health systems needed. Coming out of it, EvidenceCare had to make sure its solutions supported good clinical care and did something for the hospital’s bottom line. That shift produced AdmissionCare: a tool that helps physicians determine the appropriate bed status for a patient and capture the medical-necessity documentation payers require, without adding a step to their process. AdmissionCare now runs at dozens of leading health systems, improving admission workflows and revenue capture at the same time.
Building the BetterCare Platform
Two acquisitions extended what the platform could do.
CareGauge (2021)
The acquisition of Healthcare Value Analytics brought CareGauge onto the BetterCare Platform. It gives physicians real-time visibility into a patient’s care utilization across imaging, labs, medications, and length of stay, along with the associated costs, at the point where a physician can still act on it.
CareInsights (2025)
The acquisition of Agathos brought CareInsights onto the platform. It delivers individualized, peer-benchmarked feedback to physicians that drives improvement across the quality, throughput, and utilization metrics health systems are already being measured on.
Together, AdmissionCare, CareGauge, and CareInsights give health systems a single platform built around the initiatives already at the top of their list, instead of three separate point solutions competing for a physician’s attention.
Partnerships built for a demanding market
Over the past few years, EvidenceCare has built partnerships with physician management groups, clinical content vendors, performance improvement consulting groups, and revenue cycle management companies. Those partnerships let EC’s partners, and the health systems they serve, put complementary solutions to work without adding vendors to manage.
“When we started, the idea was simple: give physicians the right information at the moment they’re deciding what’s best for a patient. Ten years later, watching physicians use our tools to make better care decisions for their patients every day is what I’m proudest of.” — Dr. Brian Fengler, Chief Medical Officer and Co-Founder, EvidenceCare
“Ten years in, the results speak for themselves. Health systems are tired of point solutions that overpromise and underdeliver. Our job has been to prove that better clinical decisions and healthy margins aren’t a trade-off, and that when you put the two together and lean into the people, process, and technology to drive value realization, doctors, patients, and health systems win.” — Bo Bartholomew, CEO, EvidenceCare
What the next ten years look like
EvidenceCare’s mission hasn’t moved: fit tools naturally into a physician’s workflow and help them make a better decision for the patient in front of them. What’s changing is how much more the platform can do inside that workflow. The team is investing across every product, data set, and insight to bring AI into the platform thoughtfully, with governance health systems can trust before they put it in front of clinicians.
The next decade of clinical performance improvement starts now.
